MyConnectSV is a secure website packed with tools to help you find your way to a permanent home. It is connected to Santa Clara County’s Homeless Management Information System (HMIS), and can give you information about your service history. The MyConnectSV tool also makes communication with service providers easier, helping them better understand your needs and connect you to the right services.
