The goal of system performance measurement is to help the Continuum of Care (CoC) assess the effectiveness of our entire system of care and how it operates as a system, rather than just evaluating each program independently.
Performance measurement helps us understand how our system functions, what works, what doesn’t work, and what are the gaps and challenges to be addressed.
